\n \n\n \nThe role you'll contribute:
\n \n\n \nPrepares and organizes pharmacy medications by name and dosage, as listed on patient orders.
\n \n\n \nThe value you'll bring to the team:

\n \n\n \nCompletes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems
\n \n\n \nProficiently verifies/enters medication orders and proactively communicates with medical staff to resolve problem medication orders
\n \n\n \nProvides patient centered care services
\n \n\n \nInitiates and facilitates appropriate therapy based on age, disease, and drug considerations
\n \n\n \nPromotes cost-effective drug therapy and actively participates in stewardship activities and restricted medication/surveillance programs
\n \n\n \nFacilitates medication safety practices
\n \n\n \nIdentifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors
\n \n\n \nDocuments pharmacist interventions and appropriately documents in the patient medical record
\n \n\n \nClinical Pharmacy Services
\n \n\n \nProvides support to direct patient care programs
\n \n\n \nProvides accurate, safe, timely, and appropriate medication therapy based on patient need
\n \n\n \nPlaces practice priority on the delivery of patient-centered care
\n \n\n \nSupports practice in decentralized areas
\n \n\n \nCollaborates with other pharmacy staff
\n \n\n \nCollaborates with care providers and Pharmacists to optimize care and improve services
\n \n\n \nThe expertise and experiences you'll need to succeed:
\n \n\n \nMinimum qualifications :
\n \n\n \n· Bachelors and 1 years experience
\n \n\n \n· Texas Licensed Pharmacist
