The Coordinator of Care RN at MJHS is responsible for planning, implementing, and continuously evaluating home care services for patients recovering from illness, injury, or surgery. This role involves clinical patient assessment, medication management, care plan coordination, and communication with patients, families, and healthcare professionals. The position requires strong clinical judgment, autonomy, and collaboration within an integrated care team, supported by a current RN license and nursing experience.

At MJHS , our dedicated staff provides a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.
In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Your excellent communication, documentation and time management capabilities will be essential to your success. And all along the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job right.
Excellent clinical assessment skills
Strong ability to solve problems independently and interact with an integrated team
Current NYS RN license and registration
Bachelor’s degree in nursing preferred
Minimum of one year acute medical-surgical nursing experience
CHHA experience preferred
